Care and referral details
If you provide care needs, health context, Support at Home, restorative care or referrer information, we use it to understand the enquiry and direct it to the right Avanza Care team member.
Consent and authority
If you submit information for someone else, such as an older family member, client or patient, please make sure you have a reasonable basis to share those details.
How information is used
We use enquiry and referral information to contact you, understand the support being requested, coordinate a team response and keep a record of the request. We do not sell enquiry information.
Sharing and storage
We may share details within Avanza Care or with service partners where needed to respond to the request. Part of the Avanti Group — your choice of providers. We take reasonable steps to keep enquiry and referral information secure and accurate.
Corrections
You can ask us to update contact details, correct information or talk through what was submitted.
