Many people contact us on behalf of a parent, partner, client or patient. We only ask for the information needed to understand the request and work out the appropriate next step.

Information we collect

This may include your name, contact details, location, enquiry type, preferred service area, referral notes and the best way to follow up.

Health, care and referral details

If you provide care needs, health context, Support at Home, CHSP, restorative care or referrer information, we use it to understand the enquiry and direct it to the right Avanza Care team member.

Consent and authority

If you submit information for someone else, such as an older family member, client or patient, please make sure you have a reasonable basis to share those details. We may need to confirm consent, authority or the person's preferences before discussing care options in detail.

How we use and share information

We use enquiry and referral information to contact you, understand the support being requested, coordinate a team response and keep a record of the request. We do not sell enquiry information. We may share details within Avanza Care, the Avanti Group or with service partners where needed to respond to the request.

Storage and corrections

We take reasonable steps to keep enquiry and referral information secure and accurate. You can ask us to update contact details, correct information or talk through what was submitted.

Response expectations

A care planner will usually follow up during business hours. If the matter is urgent or someone may be at immediate risk, please contact emergency services or the person's existing health provider rather than using this website.
